This is a work-from-home position with travel required within  Florence, SC County territory/community.   Mileage is reimbursed for travel.   Summary: The LPN Care Coordinator is a licensed practical nurse, who ensures patient navigation and oversight for Tier 2 and Tier 3 participants in the Medically Complex Children’s Waiver program. This is achieved by managing client caseloads in an assigned geographical area within South Carolina, and working in conjunction with the assigned RN case manager to ensure the client’s needs are met in the home setting. This position will require information gathering (both in person and telephonically), implementation of a service plan/plan of care created by the RN case manager, collaborating with other members of the client’s healthcare team, assessing current services, and identifying new needs. The LPN Care Coordinator will operate within the LPN Scope of Practice outlined by the SC State Board of Nursing. This position will also require travel.

Salary and RVU Bonus Structure compensation is based on productivity.
