Overview

This is a work-from-home position with travel required within 
Florence, SC County territory/community.  
Mileage is reimbursed for travel.

 

Summary:

The LPN Care Coordinator is a licensed practical nurse, who ensures patient navigation and oversight for Tier 2 and Tier 3 participants in the Medically Complex Children’s Waiver program. This is achieved by managing client caseloads in an assigned geographical area within South Carolina, and working in conjunction with the assigned RN case manager to ensure the client’s needs are met in the home setting. This position will require information gathering (both in person and telephonically), implementation of a service plan/plan of care created by the RN case manager, collaborating with other members of the client’s healthcare team, assessing current services, and identifying new needs. The LPN Care Coordinator will operate within the LPN Scope of Practice outlined by the SC State Board of Nursing. This position will also require travel.

Responsibilities

Essential Duties and Responsibilities:

  • Conducts monthly telephonic reviews for an assigned number of MCCW participants to gather information, assisting in evaluating current services, and identifying new needs.
  • Conducts in person on site visits with the assigned MCCW clients and their responsible party twice yearly to update the current plan of care, evaluate current goods and services the client is receiving, and to identify any new needs or barriers to care.
  • Collaborates and case conferences with the RN Case manager as needed for new identified needs that may require a service plan or plan of care update.
  • Attends and participates in monthly all staff meetings, as well as case conferences with their assigned RN case manager monthly and as needed.
  • Maintains availability to family/responsible party and providers during normal business hours to coordinate any required services.
  • Manages transportation/travel expenses.
  • Maintains productivity requirements as outlined by the leadership team of SC Solutions and submits all required productivity reports timely.
  • Will have access to and will and manage PHI in accordance with federal and state requirements (HIPAA, HITEC).

Qualifications

Qualifications:

  •  A Minimum of three (3) years of recent clinical pediatric experience is required.
  • Previous work experience with a Medicaid program is preferred.
  • Bilingual is preferred.
  • Ability to travel locally with a valid drivers’ license and automobile insurance.
  • Criminal background check as required by the state where business is conducted
  • Negative TB test

 

Education and/or Experience:

  • Graduate of an accredited LPN program.
  • A minimum of three (3) years of Pediatric Nursing experience.
  • Pediatric Home Care experience is strongly preferred.

 

Certificates, Licenses, Registrations:

  • Current LPN licensure in states where business is conducted.
  • Current BLS certification
  • Current CPR certification

 

Computer Skills:

 

Proficiency using software programs such as MS Word, PowerPoint, Excel, Outlook, and Google suite

 

 

Environmental Factors/Physical Demands:

 

Work is performed in an office environment. While performing the duties of this job, the employee is regularly required to have the ability to maintain active customer and employee communication; access, input and retrieve information from the computer system; enter alpha-numeric data into a computerized system often while listening on the telephone. May be subject to repetitive motion such as typing, data entry and vision to monitor. May be subject to bending, reaching, kneeling, stooping and lifting up to thirty (30) pounds.
